Pre-Check-In to your next Appointment from your Computer or Mobile Device
What is pre-check-in?
If you have granted IHG permission to contact you via phone or email, you will receive and electronic pre-check-in link by email or text prior to your next appointment. The pre-check-in process will prompt you to provide some basic information that you may typically provide when you check-in at our office for your appointment. By completing the pre-check-in, your check-in process in our office on the day of your appointment will be expedited. During the pre-check-in process, you will have the opportunity to review and update some of your patient information on file at our office.
What you may need to complete your pre-check-in?
To complete the pre-check-in for yourself or for another patient as a parent or legal guardian, you may need the following information:
- Date of birth
- Zip code
- Phone number
- Email address
- Address
- Driver's license or photo ID (in some instances)
- Pharmacy name and address
- Insurance Provider
- Insurance Card (in some instances)
- Emergency contact information: Name, phone number
- Information specific to patient to complete any screenings or questionnaires
